POSITION OVERVIEW:
Under the direction of the Executive Director and Program Manager, the Case Manager is responsible for programs administered by Community Housing and Shelter Services (CHSS).

The Case Manager is primarily responsible for providing counseling and case management services to those experiencing homelessness and/or are at imminent risk of becoming homeless.  These services encompass supportive service programs administered by CHSS.  He/She is employed by the CHSS Board of Directors and is directly responsible to the Executive Director and Program Manager.

This position requires a sensitive and caring individual who is able to develop trust with their clients. He/She must be willing to work with persons of diverse cultural backgrounds, allow flexibility in their work and work within a team approach.

ESSENTIAL JOB FUNCTIONS:
  • Screen, interview, and determine eligibility for services.
  • Coordinate intake and individualize needs assessments, interviewing participants to develop short and long-term goals that address barriers to obtain services and housing.
  • Provide assessment of needs identifies and defines problem areas and assists clients in the problem-solving process.
  • Provide advocacy, information, and referrals to families/individuals as necessary.
  • Prepare correspondence as needed for clients.
  • Maintain regular contact with property owners, apartment managers and realtors for current available housing.
  • Conduct all aspects of work with complete courtesy and respect to clients.
  • Maintain daily records and notes of client contacts and services provided.
  • Travel to other referral agencies, other program sites and home visits as necessary to accomplish, intakes, and follow-ups.
  • Work collaboratively with providers and community-based organizations.
  • Carry and effectively manage a caseload.
  • Provide supportive services in a culturally responsive and non-judgmental manner using a strengths-based approach.
  • Other duties as assigned.
MINIMUM QUALIFICATIONS:
  • Understand and promote the Vision/Mission Statement CHSS.
  • Knowledge of social work ethics and practices
  • Ability to work with individuals of diverse cultural backgrounds who may be low-income, particularly with those experiencing homelessness or at-risk of homelessness.
  • Ability to work independently and use discretion in making decisions.
  • Ability to learn resources in the community in order to provide information and referral services to meet client needs.
  • Make decisions permitting essential functions to be carried out in an accurate, timely manner.
  • Understand, interpret and apply rules and regulations as they relate to assigned area of responsibilities.
  • Communicate effectively, orally and in writing.
  • Maintain confidential information according to the legal standards and/or County regulations as required.
  • Required computer skills: Intermediate experience with data entry and word processing; be capable of operating routine office equipment; possess a working knowledge of MS Office applications and understand how to navigate through web-based applications.
  • A positive attitude that is flexible and adaptable to a changing environment.

EDUCATION/EXPERIENCE:
  • Working towards B.A or possessing an A.A. degree in Human Services, Social Welfare or related field or study.  Experience and skill in these fields may be substituted for academic and/or formal training.
  • One year experience in a public or private social services and/or disability or housing agency, with experience in social work, outreach, social services and/or disability-based government program case management programs, and/or housing case management for multi-barrier populations.
  • Proof of education is required.
OR
  • Years of experience in a public or private social services and/or disability or housing agency, with experience in social work, outreach, social services and/or disability-based government program case management programs, and/or housing case management for multi-barrier populations may be substituted for the required education on a year-for-year basis.
LICENSE:
  • Must have a vehicle for daily use and in-state insurance with your name on the policy.
  • Applicants must possess and maintain a valid California Driver's License and remain free from repeatable preventable accidents.
DESIRABLE QUALIFICATIONS:
  • Bilingual, Spanish

COMPENSATION/SALARY:
$19.27 per Hour

BENEFITS SUMMARY
  • Eligible for benefits for Full-Time Employee after 3-month introductory period.
  • Insurance: Medical, Dental and Vision offered up to $600 monthly costs (any overage costs will be paid for by the employee).
  • Vacation Accrual
1-4 years                         10 days per year (2 weeks)
5-9 years                         15 days per year (3 weeks)
10 years and thereafter   20 days per year (4 weeks)
  • Sick Leave Accrual- 1 day per month
  • Retirement Plan Available after 1 Year of employment
